The PWS-12 was a biplane trainer designed and developed by Podlaska Wytwórnia Samolotów (PWS). It entered production as the PWS-14.

The part number is commonly listed as PWB PWS-14 - if you like I can send you a pic of the board to compare with what you are looking at. I have a blown one and a new one.1
